The Jackson-Madison County Regional Health Department announces the launch of a new program called Community Health Access and Navigation in Tennessee (CHANT).
According to a press release, CHANT is a voluntary educational program designed to give local families the education and tools they need to be successful. Our staff will teach families how to navigate the healthcare system and how to use available resources.
Participants will learn about a variety of topics including preventative care, vaccinations, and the importance of Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) services. “We engage families through home visits, calls, and office visits,” CHANT Program Manager Lindsey Nanney, RN, said. “We make sure you have the right tools and education to raise your family.”
The Children and Youth with Special Healthcare Needs (CYSHN) program is a part of CHANT that focuses on families with children who have physical disabilities. It helps provide extra guidance and education as well as possible financial support. Our staff can coordinate services for children and youth with special healthcare needs and connect families with others going through similar situations.
In Madison County pregnant and postpartum women, all children under age 5, and people eligible for TennCare Kids can qualify for the CHANT program. Children diagnosed with a physical disability which requires medical, surgical, dental, or rehabilitative treatment may be eligible for CYSHN. Family size and income is a factor.
